Before Your First Visit

To prepare for your first visit, you may download, print and fill out the following forms:

3. patient medical history (pdf, 1 page, 19 KB)
4. release for medical information (pdf, 1 page, 13 KB)
5. general info and payment policy (pdf, 5 pages, 226 KB)

(You will need a .pdf reader on your computer. In the unlikely event that you do not have one installed, you may get adobe reader.)

First Visit

On the day of your appointment, please:

• wear loose fitting, comfortable clothing
• if needed, take pain medication(s) 30 minutes prior to visit

Attendance Policy

Please arrive on time for your appointment. Your active participation is required to receive the maximum benefit from your rehabilitation program.

If you find that you cannot keep your appointment, please call to reschedule as soon as possible. Patients who fail to cancel an appointment at least 24 hours in advance will be charged $25.00.

Remember, compliance with your physician’s rehabilitation prescription is important. Frequent cancellations or no shows by any patient will be reported to the referring physician.

In the event of severe weather, you may call 336.918.7476 to learn about delays or closings. We will do our best to notify clients if we are in the office and must close early.
