Before Your First Visit

To prepare for your first visit, you may download, print and fill out the following forms:

1. health care consent (pdf, 2 pages, 20 KB)
2. patient info (pdf, 1 page, 12 KB)
3. general info and payment policy (pdf, 5 pages, 233 KB)

Alternatively, you may download one larger file which contains all documents (pdf, 8 pages, 270 KB).

(You will need a .pdf reader on your computer. In the unlikely event that you do not have one installed, you may get adobe reader.)

First Visit

On the day of your appointment, please:

• wear loose fitting, comfortable clothing
• if needed, take pain medication(s) 30 minutes prior to visit

Attendance Policy

Please arrive on time for your appointment. Your active participation is required to receive the maximum benefit from your rehabilitation program.

If you find that you cannot keep your appointment, please call to reschedule as soon as possible. Patients who fail to cancel an appointment at least 24 hours in advance will be charged $25.00.

Remember, compliance with your physician’s rehabilitation prescription is important. Frequent cancellations or no shows by any patient will be reported to the referring physician.
